As part of a multi-disciplinary team your role as a Personalised Care Manager (Social Worker) will be to work with mental health service users and carers in developing person centred and ‘recovery’ focussed packages of care which meet users’ own unique needs and which are achievable within available finances.
You will work with the individual (and in consultation with their family carers where appropriate) to identify and source the most appropriate care package/community placement, empowering the individual to take control and responsibility through promotion of self-directed support and Direct Payments.

A suitable Social Worker will:
- Be a fully Qualified Social Worker (Degree / Diploma in Social Worker)
- Have current NISCC registration (Part one) - or eligible for registration on appointment
- Hold a current full driving licence and access to a suitable vehicle for business purposes
