We are recruiting a Band 4 Social Care Coordinator, who will work with and provide support to an Older People Community Social Work Team to build therapeutic working relationships with elderly service users to address the specific challenges of the aging process by promoting independence, autonomy, and dignity in later life.
Under the direction and guidance of the social workers, the social care co-ordinator will work to ensure the implementation of person centred interventions, activities and or services to support and meet assessed need and to achieve desired outcomes, as well as contribute to the monitoring of the wellbeing of service users in accordance with the social work intervention plan.
You will have:
- 5 GCSEs at grade C and above inc English Language
or
- NVQ Level 3 in Care (or equivalent)
and
- a minimum of 2 years paid or voluntary experience working with older people and/or people with physical and health related disabilities
- current NISCC registration
