We are recruiting two Assistant Care Managers for full time, ongoing temporary assignments in Belfast.
The Care Management Service assesses the needs of the physically disabled and sensory impaired to provide appropriate packages of care.
A Care Management Assistant will helps the team with the coordination of care packages by liaising with statutory, voluntary and private sector car providers. You will monitor and review domiciliary care packages, residential / nursing home placements and will be involved in the completion of financial documentation.

You will have:
- A minimum of 3 years paid experience working with older people and/or people with physical health/disabilities in a social care setting.
- Have 5 GCSE’s (Grades A - C) or equivalent.
- Hold a full current driving licence valid in the UK - this criteria will be waived in the case of an applicant whose disability prohibits driving but who is able to organise suitable alternative arrangements.
