As a contracted supplier of Agency Workers to the Belfast H&SC Trust, we are recruiting a Band 4 Social Work Assistant for a full time, temporary assignment based in an Acute Hospital setting in Belfast.
Under the direction of the Hospital Social Work Team you will be responsible for assisting in the flow of discharge from hospital. Your main role will be communication and support with the patient and their family regarding safe and compassionate discharge pathways from the acute site.

You will have:
- Five GCSE’s including English Language (Grades A-C) or equivalent
OR
- NVQ Level 3 in Care or equivalent
AND
- A minimum of 2 years paid or voluntary experience working with Older People and or people with physical health and disabilities
- Ideally experience/knowledge of the hospital discharge process
